The Community Health Foundation is organized and will operate for the exclusive purpose of engaging in charitable, educational and scientific activities to specifically enhance the quality of life for members of the communities of Stark, Wayne and Tuscarawas Counties in Ohio, by educating, promoting and supporting the health of members of those communities. Its assets shall be devoted to initiatives and programs that promote physical, emotional and mental health of members in that community and also initiatives and programs, which further the preventive health care of those members in the communities.

Purpose:

• Develop and implement a strategic philanthropic plan with the assistance of community input, collaboration from local resources and a thorough assessment of local needs
• To make valuable contributions and a significant impact on the quality of life for members of the communities we serve
• Create and foster a working and funding environment which is indicative of our principles and code of ethics
